Driver's License Number, Vehicle Make, Vehicle Model, Vehicle Year, Vehicle Color, Vehicle License Plate, Registered Owner, Drivers Relationship to Owner, Owners Phone Number, Insurance Policy Number, and Insurance Company Name.
